DIRECT SUPPORT and ADMINISTRATIVE POSITIONS

 Home Community Provider – On Call – Family Services – Starting at $15.00/hr!!
The Home/Community Provider implements a prescribed habilitation plan for eligible participants while providing routine direct care either in the home or community.  This position works closely with the family or guardian and the FRA Home/Community Services Manager to design, implement, and modify consistent intervention strategies that can ultimately be maintained by the family or guardian in their daily routines.

Day Habilitation Manager – Discovery Center
The Day Habilitation Manager has responsibility for the provision of coordinating services with families and individuals in the center-based day habilitation setting.  The Day Habilitation Manager will work with the Home/Community Services Program Manager to assist with coordinating services. The Day Habilitation Manager assists with the identification and assessment of families in need of day habilitation services, and assures that services meet federal, state, and agency guidelines and regulations. The Day Habilitation Manager, in coordination with the Home/Community Services Manager, is responsible for recruiting, orientation, training and supervising staff within the day habilitation setting. The Day Habilitation Manager will assist with coordination of program transportation and provision of services. (Closes Monday Sept. 25, 2017)

Day Habilitation Provider – Full Time – Discovery Center – Starting at $15.00/hr!!
The Day Habilitation Provider is responsible for working as a team member in assisting participants with developmental disabilities in their recreational activities and daily living skills, implementation of participant’s POC’s, promoting each participants freedom of choice and independence, maintaining participants files, transporting participants in the community, and documentation of progress. The Day Habilitation Provider will ensure that activities and services are provided appropriately and in accordance with participants needs.

Chore Provider – On Call Position – Senior Services – Starting at $15.00/hr!!
The Chore Provider performs tasks to assist a participant in maintaining his/her independence while allowing them to remain in their home.  The Chore Provider performs household tasks that individuals cannot perform themselves. (Opened until filled)

Respite Provider – On Call Position – Senior Services – Starting at $15.00/hr!!
The Respite Care Provider provides short term, direct care for eligible participants.  The purpose of this care is to provide relief for family members, parents or guardians to enable them to attend to personal matters.  The Respite Care Provider has a demonstrated interest in working in direct care position serving the emotional, physical and health needs of eligible participants.

Support Living Specialist – Starting at $15.00/hr!!
Residential: Provides and/or coordinates supports that are necessary to enable FRA participants to live as independently as possible.  This includes assisting in the planning, implementing and documenting of the Plans of Care (POC’s), ensuring the well-being, safety and quality of life of program participants, providing guidance and training that facilitates independence and recognizing and respecting the rights of the participant.

Employment Specialist II – Part Time Position or On-Call – Starting at $17.00/hr!!
The Employment Specialist II plans, implements and evaluates systematic consumer training programs, which are designed to maximize individual potential for meaningful independent employment in the community.  Training activities emphasize appropriate social behavior, skills acquisition, and self-direction.  The Employment Specialist II provides assistance in both short and long term planning for the work-training program.

Project Manager manages the execution of the Performance Work Statement (PWS) of his/her assigned contracts.  Operational responsibilities include the management of quality control, training, supervision, liaison duties, budgeting, customer services, and oversight of assigned PWS tasks. This position generally requires 24-hour availability.
